This includes receipts, unused credit card offers, bank statements, etc. • Order and review copies of your credit report at least annually.

YOUR FINANCIAL PRIVACY RIGHTS Important definitions: • We and us means Citizens Bank of Ada. • You and Yours means all of our consumers who have a continuing relationship with us, including deposit and loan accounts. • Nonpublic Personal Information means any personally identifiable financial information that we collect from you. For example, information regarding your account balance & payment history. It does not include information that is publicly available, such as from telephone directories or government records. • Affiliate is a company that controls, or is controlled by at least 25% or is under common control with another company. • Nonaffiliated Third Party is a company that is not an affiliate of ours.
What Information We Collect We may collect “nonpublic personal information” about you from the following sources: • Information we receive from you on applications or other loan and account forms. • Information about your transactions with us or others. • Information we receive from third parties such as credit bureaus.
What Information We Disclose We are permitted under law to disclose nonpublic information about you to other third parties in certain circumstances. For example, we may disclose nonpublic personal information about you to third parties to assist us in servicing your loan or account with us to government entities in response to subpoenas, and to the credit bureau. We do not disclose any nonpublic personal information about you to anyone, except as permitted by law. If you decide to close your account(s) or become an inactive customer, we will continue to adhere to the privacy policies and practices described in this notice.
OUR SECURITY PROCEDURES We also take steps to safeguard customer information. We restrict access to your personal and account information to those employees who need to know that information to provide products or services to you. Employees who violate these standards will be subject to disciplinary measures. We maintain physical, electronic and procedural safeguards that comply with federal standards to guard your nonpublic personal information.
KEEPING UP –TO-DATE WITH OUR PRIVACY POLICY Citizens Bank of Ada will provide notice of our privacy policy annually, as long as you maintain an ongoing relationship with us. This policy may change from time to time. You can come by our offices and pick up a copy during regular business hours.
